Jazmin Dietrich of York died at Reading Hospital the same day as the crash, according to state police in Lancaster.
The crash happened on PA Route 222 North just north of Dry Tavern Road in Brecknock Township at 8:33 a.m. on March 30, investigating Trooper Joshua Mease detailed in the release.
Jazmin was wearing a seatbelt in front passenger's seat of a BMW driven by 51-year-old William De La Cruz of York when the vehicle experienced an unknown mechanical failure, slowing down in the left lane, Mease explained. That's when 31-year-old Jesus Barrera's Porsche SUV rear-ended the BMW; both vehicles suffered "major damage," Mease said.
Immediately after this crash, 25-year-old Shane Wright of Ephrata struck the Porsche with his Nissan SUV, according Mease.
Wright, his passenger, De La Cruz, and Barrera all suffered "moderate injuries" and were also taken to Reading Hospital, PSP stated in the release.
The roadway was closed for over five hours and the investigation is ongoing.

Jazmin was a Carlisle native who attended Carlisle High School, Spring Grove Area High School, and Purdue Global, according to her social media. Over the years she worked at Hanover House of the Homeless and the West Manchester Diner.
Jazmin was an organ donor according to her community, which has been mourning its loss on social media.
One of Jazmin's last posts on social media was dedicated to her friends and family, wishing them a Happy Easter, which she turned out to never see.
Funeral and memorial service details have not been released at the time of publishing.
